Hellò Sebloders, I would like to create a form where I can choose a institution from a file and then add the fields that I need. I currently use successfully plugin "select dynamic cascade" and I try my institution by the province and the city, but now I have another need: 

If I do not know the province and the city would like to be able to choose the institution through the name or part of it, or by the key of the record. 

Therefore, I have to choose from 3 types of research. 

Independent of the research I'd like always to see and save the personal data of the institution (name, address, zipcode, city, province) together with all the information required on the form. 

So,I can do further research using "search and type" of seblod without problem.
